TO BE USED ONLY BY CREIGHTON FACULTY/STAFF WHO ARE AUTHORIZED TO HIRE CREIGHTON STUDENTS

IMPORTANT:

  • If you wish to hire, give a pay raise or terminate a Creighton student, complete this PR-2-S form and mail or email (tmcgra@creighton.edu) to the Student Employment Office, Harper Hall rm 2054.  To insure integrity of this transaction, a scanned copy of processed form will be emailed back to hiring supervisor.
  • Withholding Tax Allowance certificate (IRS W-4) and Employment Eligibility Verification (Immigration form I-9) must accompany this authorization, if student a new hire or has not been employed by the university within the last year. W-4 and I-9 can be downloaded from the web. Go to Student Employment home page and look under "hiring materials".
  • Length of employment is current fiscal year only.
  • International  students must obtain valid Social Security number.  At time of hire, they must show  their unexpired foreign passport with US VISA, I-94 and Immigration form I-20 to the hiring supervisor.  Supervisor should copy these documents and attach to the I-9 they send to the Student Employment Office.
  • All grant-funded positions (200000 # series funds) must be approved by the Controller's Office prior to hire.

Federal Work Study students may also work under your departmental funds without jeopardizing their FWS award.


The Student Employment Office reserves the right to refuse to process this form if any of the following conditions are not met:

  1. PR2-S form is not signed by the authorized hiring supervisor.
  2. The signature is not legible, or the person is not recognized as having the authority to hire.
  3. The supervisor has not included a valid Creighton e-mail address. The Student Employment Office will e-mail supervisor a scanned copy of processed PR-2-S.
