How to apply for a Fall Break Service Trip
1) Check out all the great sites we are going to this year. All applications are to the general Fall Break Service Trip Program, not for a specific site. We will take into consideration your preferences, but offer no guarantees of where you will be placed.
2) Download the “FBST Essay Questions” document, fill it out, and print it.
3) Download the “FBST Emergency Form” document, fill it out, and print it.
4) If you are under 19, download the "Under 19 Parent Consent and Release Form" document on our website. You must have a parent or guardian sign it.
5) If you are in need of a scholarship, please contact Kelly Orbik or download the “Scholarship Application” and email to Kelly. Limited scholarships are available and based on need.
6) For the first time, you can now do most of the application online! Fill out this part of the application and hit the “submit” button.
7) After you have submitted your application online, you will receive a confirmation email to let you know we received your application, but in order to complete the application process, you need to…
8) Bring the following documents to the CCSJ Office in the Harper Center (2067)
-Completed FBST Essay Questions
-Completed FBST Emergency Form
-Your payment of $225 (We accept cash, check or credit card)
-If under 19, signed copy of the Under 19 Parent Consent and Release form
-Don't forget to sign up for an interview in our office before Sept 12
Applications are due Sept 8, any application received after will be marked late.
If you are accepted and you decide not to participate in this trip, before the final drop date of Sept. 17, you will be charged a $40 cancellation fee. If you are accepted and you decide not to participate in the trip after Sept. 17 your fee will be non-refundable. This is necessary because the CCSJ allocates site payments based on your acceptance to the program, which may have precluded someone else from participating.
