The family member who is first referred to the clinic is identified as the proband. The proband (the proband’s parent, in the case of a minor) will be asked to be a partner in the evaluation by gathering the family history for at least three generations. Some families have much of the information in a family bible or a genealogy, others may pass on the information by word of mouth. Often it is helpful to talk with older members of the family who are aware of most of the genealogy and medical history. The proband also will be asked to assist with obtaining permission for release of medical records from family members who have had cancer. If a family member who had cancer is deceased, the next of kin will need to be asked to sign the release form. When older medical records have been destroyed, a death certificate may be helpful.
