Study Abroad Forms
Students should plan to submit all forms and documentation to the OIP front desk or by regular mail. Incomplete forms are not accepted. The Application for Study Abroad Approval is due on October 1 for Spring semester participation and March 1 for Summer, Fall, or Academic Year participation. All other forms and documentation listed below are due in the OIP on, or before, the "Forms Deadline" (December 1 for spring semester participants, May 1 for Summer, Fall, and Academic Year participants.) As outlined in the
Study Abroad Policies and Procedures, students who do not have complete files by the "Forms Deadline" will NOT receive final study abroad approval and risk losing academic credit and/or financial aid for their study abroad experience. NOTE: Faxes are not accepted.
The forms listed below will be distributed during the Pre-Departure Orientation and can also be obtained upon request from the Study Abroad Coordinator.

Copy of the Photo Page of your Passport | Passport copies should be legible. Be sure to sign your passport before copying. | December 1 for Spring semeser participation May 1 for Summer, Fall, or Year participation |
Copy of the Acceptance Letter from your program sponsor | When you have received your acceptance packet from your study abroad program sponsor or host institution, please make a copy and submit it to the OIP. Do not submit the original letter as you may need it for visa purposes when entering your host country. | December 1 for Spring semeser participation May 1 for Summer, Fall, or Year participation |

The ISIC card includes international health insurance and provides student discounts within the host country. This card is required for students participating on Creighton-sponsored programs (Encuentro Dominicano, University of Limerick, Sophia University, Manneheim University, Faculty-Led Programs, and Project CURA). Students participating on Associate Programs are strongly encouraged to purchase the ISIC card. | December 1 for Spring semeser participation May 1 for Summer, Fall, or Year participation |
