Project CURA Forms

Required Forms & Documentation for Students Participating in Project CURA

Students should plan to submit forms directly to the group leader(s) by April 30th. As outlined in the Study Abroad Policies & Procedures, students who do not have complete files by the deadline indicated will NOT be eligible to participate on the program. NOTE:  Faxes are not accepted. 

Note to Group Leaders:  In addition to the documents listed below, each group leader will be required to submit a day-to-day itinerary, hotel contact information, and information for any additional in-country transportation arrangements.

Forms are not yet available online.  To obtain the forms listed below, please contact your group leader or the Study Abroad Coordinator.

Copy of Photo Page of Passport

Passport copies should be legible. Be sure to sign your passport before copying. 

 

Assumption of Risk & Release Waiver

Health Disclosure Form

Medical Approval Form

Click on the form name to open a pdf file.  Please print the form and submit it to your group leader when complete.  This form must be completed by CU Student Health or your healthcare provider and submitted to your group leader by April 15thBe sure to plan the visit to your doctor in advance and don’t wait until the last minute as there are many students going abroad at the same time you are and will also be requesting appointments.                                            

 

Flight Information Form

Students may submit a flight itinerary in addition to this form, but a flight itinerary does not replace this form.  Click on the form name to open a pdf file.  Please print the form and submit it to your group leader when complete. 

 

International Student Identity Card Application (ISIC)

The ISIC card includes international health insurance and provides student discounts within the host country.  This card is required for students participating Project CURA trips.
