Required Forms & Documentation for Individuals Traveling Abroad to Work, Volunteer, Teach, Intern, or Conduct Research
Many students, faculty, and staff travel abroad to work, volunteer, teach, intern, or conduct research. If you are traveling under the auspice of Creighton University, you should plan to alert the Office of International Programs regarding your travel plans and complete the forms listed below. We hope that these actions will help to ensure that you have a safe and successful experience abroad.
Please submit the documents below at least one month prior to your departure.

The 2nd page must be completed by CU Student Health or your healthcare provider and submitted the OIP at least one month prior to your departure. The ISIC card includes international health insurance and provides student discounts within the host country.
