Required Documentation and Forms

STUDY ABROAD

If you have completed an Application for Study Abroad Approval, you will need to submit the forms and documentation listed in this section. Students who do not submit the required forms by the "Forms Deadline" will not receive final approval from the OIP and risk losing academic credit and/or financial aid for their study abroad experience. 

ENCUENTRO DOMINICANO

If you have submitted an Encuentro Dominicano Application, you will need to submit the forms and documentation listed in this section. Students who do not submit the required forms by the "Forms Deadline" will not receive final approval from the OIP and risk losing academic credit and/or financial aid for their study abroad experience. 

FACULTY-LED PROGRAMS ABROAD (FLPA)

If you are participating on a summer faculty-led trip abroad, you will need to submit the forms listed in this section.  Forms should be turned in to the College of Arts & Sciences Dean's Office on or before April 1st.  Students who do not submit the required forms by the deadline will not be eligible to participate on the program.   

PROJECT CURA

If you are participating on a Project CURA trip, you will need to submit the forms listed in this section directly to your group leader.  Forms turned in directly to the OIP will not be accepted. Check with your group leader for deadlines. Students who do not submit the required forms by the deadline will not be eligible to participate on the program.   

INDIVIDUALS TRAVELING ABROAD

If you are a Creighton student (undergraduate, graduate, Medical, Pharmacy, OT, PT, Dental) and traveling abroad to work, volunteer, or conduct research, you are required to submit the forms listed in this section to the Office of International Programs (OIP).  You should also check with your respective college for information about any additional items that may be required of you.  Forms are due in the OIP no later than 4 weeks prior to your departure.  Faculty and staff traveling abroad to teach, work, volunteer, conduct research, or attend conferences are asked to submit the Assumption of Risk and Release Waiver Form, ISIC Insurance Application, and Flight Information Form.
