FLPA Forms

The FLPA Application for Participation should be submitted directly to the Office of International Programs (Lower Becker G-25) on or before February 15th.  Additional Forms and Documentation should be submitted to the College of Arts and Sciences Dean's Office (Admin Building, Rm 240) by April 1st. As outlined in the Policies, Procedures, and Eligibility Requirements for Study Abroad, students who do not have complete files by the indicated deadline will NOT be eligible to participate on the program. NOTE:  Faxes are not accepted. 

Note to Faculty Leaders:  Faculty-leaders should plan to submit a copy page of their passport,  the Flight Information Form, and the ISIC Application.  In addition, the Dean's Office should receive the trip itinerary, hotel information, and in-country contact information.

Passport copies should be legible. Be sure to sign your passport before copying.  Submit signed copy to the Dean's Office.

Medical Approval Form

Complete the first page of this form.  The second page must be completed by CU Student Health or your healthcare provider and submitted to Dean's Office.  Be sure to plan the visit to your doctor in advance and don’t wait until the last minute as there are many students going abroad at the same time you are who will also be requesting appointments.

Flight Information Form

International Student Identity Card Application (ISIC)

The ISIC card includes international health insurance and provides student discounts within the host country.  This card is required for students participating on faculty-led programs abroad.  Submit the ISIC application and photo to the Dean's Office.
