SearchEncuentro Dominicano Forms
Students should plan to submit forms to the OIP in person or by regular mail. Incomplete forms are not accepted. All forms and documentation listed below are due in the OIP on, or before, the "Forms Deadline" (see below). As outlined in the Study Abroad Policies and Procedures, students who do not have complete files by the "Forms Deadline" will NOT receive final study abroad approval and risk losing academic credit and/or financial aid for the study abroad experience. NOTE: Faxes are not accepted.
The forms listed below will be distributed during the Pre-Departure Orientation and can also be obtained upon request from the Study Abroad Coordinator.

Copy of Photo Page of your Passport | Passport copies should be legible. Be sure to sign your passport before copying.

This form must be completed by CU Student Health or your healthcare provider and submitted to the OIP by the Forms Deadline. Be sure to plan the visit to your doctor in advance and don’t wait until the last minute as there are many students going abroad at the same time you are and will also be requesting appointments.

The ISIC card includes international health insurance and provides student discounts within the host country. This card is required for students participating Encuentro Dominicano.

$100 Placement Deposit | Students who have applied, interviewed, and been accepted for Encuentro Dominicano need to submit a $100 deposit to reserve a spot for the intended semester abroad. Program 'spots' are filled in order of when the deposits were received. Checks should be made payable to Creighton and are non-refundable if the student withdraws from the program at a later date. | Due after student has conducted interview and been accepted for participation |
