Creighton University Health Plan offers to you and your eligible dependents various PPO Options for medical coverage. Insurance will be effective on the first date of employment. Each option offers comprehensive benefits to protect you against the medical expenses that would accompany an illness or injury. There are differences in coverage levels and in each option. Dental Insurance
You and your eligible family members have the opportunity to enroll in the Creighton University PPO Dental Plan also called DentaBenefits. The Dental Plan is effective on the 1st of the month following the date of your employment. The Dental Plan offers you and your eligible dependents various provider options for dental coverage. Each option offers a comprehensive range of services, including:
Preventative – Class A
Restorative – Class B
Comprehensive – Class C
You can maximize your opportunity to save by using a dentist at the Creighton Dental School or in the United Concordia Network. Dental benefits are based on the type of service(s) received. Difference in coverage level reimbursement occurs under each of the Tier structures. The dental plan is administered through United Concordia Companies, Inc. (UCCI). Group Term Life/Accidental Death & Dismemberment (AD&D) Insurance
The University pays for a policy that is equal to one times (1X) your annual salary up to the next $1,000 to a maximum of $50,000. The AD&D plan provides an amount equal to the life insurance that is in force. To read additional information on the plan refer to:

Long Term Disability (LTD)
The University pays for coverage that provides a salary replacement benefit of 66 2/3% of your base monthly salary, following a 30 day elimination period. The Long Term Disability insurance will be effective on the first date of employment. Your monthly benefit will not exceed $2,500. Pre-existing waiting period applies. Employee Assistance Program
The company has adopted an Employee Assistance Program (EAP) to assist employees and their immediate family members in dealing with problems that can affect their work situation or their personal lives. The program provides the employee and/or their dependents up to 3 free confidential counseling sessions per situation, per year.

The University observes the following paid Holidays:

- Christmas Holiday Break (Christmas Eve through New Year’s Eve)
The University Official list of holidays are listed above, however, due to the unique status of the House Staff physicians and clinical faculty, holiday time-off is service dependent and is determined by the services to which the House Staff physician is assigned.
If floater days are allowed by the program, the floater days must be taken while the House Staff are assigned to primary service rotation.
