Commonly Used Forms
Depending on your particular circumstances, you may be asked by the CU Financial Aid Office to provide additional information. If this is the case, we will send you a letter requesting it. Below are the most commonly requested forms we may ask you to submit. Print out the form you need, complete it and mail, fax, or bring it to the Creighton Financial Aid Office, 2500 California Plaza, Omaha, NE 68178-0062. Our FAX is 402-280-2895. Oh, one more thing... don’t forget to sign it, or we have to mail it back for a signature. If you have questions about how to fill out a form, call: 402-280-2731 or 1-800-282 5835. - Appeal for Independent Status 2008-09
- CU Verification form 2008-09
- Family Remission Application
- Federal Work Study Agreement 2008-09
(submit only if you have been awarded and have accepted your FWS award) or if you prefer, submit an electronic FWS agreement - I-9 Federal Employment Eligibility form must be completed in person at time of hire with original documentation proving eligibility to work in the US.
- MET (Monthly Electronic Transfer) brochure and application. This payment program is administered through the Creighton Business office.
- Parents’ Estimated Yearly Income for 2008
- Student’s Estimated Yearly Income for 2008
- Statement of Non-Filing of Federal Tax Return
- Summer Aid Application 2008 or if you prefer, submit an electronic summer aid application.
The Financial aid office may require that you and your parents submit copies of your federal tax returns and W2's.
If you can't find a copy of your tax return, you can get a free transcript of your tax return from the IRS by calling 1-800-829-1040 or by completing and sending this request form #4506-T to the IRS.
