Supplier Registration
The first step to becoming an approved supplier at Creighton University starts with registration. The following forms are available for download. Completed forms can be faxed to 402.280.2691; Facilities Management strongly recommends including a 1 page company bio with the forms or mailing pertinent brochures about your firm.
- Supplier Registration Form- required
- Supplier Information Sheet -required
- Submission of a completed W-9 form
- Certificate of Insurance required for companies performing work on campus
Note: Registration form submittal does not guarantee approval. Reasons for non-inclusion can include debarment by the federal government, supplier saturation, or existing contract.
